200ft is range, or maximum distance to the receiver. Wavelength in meters = 300/frequency (Mhz) therefore wl= 300/90 = 3.3m approximately.

You can receive it on normal FM radio.
This is a spy device, so you can not talk back. You will hear both telephone users talking

well, phone line supplies the DC current along with audio signal. It is enough to power the small FM transmitter. As you never know the polarity in phone line, you need a bridge. To separate AC and DC, you need capacitor. There is probably a mistake with the switch S1 - it should be able to disconnect the circuit from the line without interrupting the conversation, but I think you can figure it out yourself.
